NAS Jacksonville Florida. When you arrive at the (Main Gate) you will proceed to the first traffic light and turn right. Go to the next traffic light turn left. Go to the next traffic light and we are located to your right. In the PSD Building #789.
To ensure the appropriate amount of time is booked to complete multiple ID Cards or transactions, please book back to back appointments. Please schedule one (1) appointment for every two (2) IDs under the same record.
Please arrive at least 5-10 minutes before your scheduled appointment time. If you arrive after your scheduled appointment time you will be placed on the walk-in schedule in a first come, first served basis.
ALL Sponsors, Spouses, and Dependent Children OVER 18 must have two UNEXPIRED forms of ID to receive a new ID card. One must be a state/government issued photo ID.
Detail: Due to a recent system upgrade, ID card offices can no longer accept the following: Student ID, Expired IDs, Veterans Identification Card (VIC). Note: Concealed Weapons Permits are not acceptable.
Primary: Military ID, Driver's License, Passport. Secondary: A second Primary ID or Social Security Card, Voter's registration card, Original Birth Ceritifcate, or Veterans Health Identification Card (VHIC). All must be unexpired.
If any changes need to be made to a record, the sponsor must be present or a valid Power of Attorney must be presented each time. Additionally, valid supporting documentation MUST be provided. Newly Married Couples must provide the following documents: O
The policy regarding the timeline for renewing your ID has recently changed. You are now allowed to renew the ID 90 days prior to the expiration date.
All Military Installations have begun scanning ID cards for Base access. If you currently hold an older ID with the SSN printed on the face of the card, it may not scan properly and will be confiscated. Renewal of the ID will then be required.
All Dependents without a Sponsor present must have a valid DD Form 1172-2 pre-printed by a DEERS station that has been signed by the Sponsor and a Verifying Official ahead of time. 1172-2 forms can be hand filled but must be notarized and be originals.
DD Form 1172-2 can now be completed online at: https://www.dmdc.osd.mil/milconnect. Sponsors with CAC or DS log-in access can digitally sign the 1172-2 and the electronic copy will be available for use in their DEERS record.
Due to recent system update, Pre-Medicare Retiree ID Cards with a previous "INDEF" end date will now be issued with an expiration date. The front expiration date will now reflect the date of the end Tricate/Begin date of Tricare for Life.
All military personnel whose CAC card has been stolen must bring a theft report from a Police Station. For Lost CAC cards, documentation from sponsoring agency reporting it lost.

When adding a spouse to DEERS, the sponsor must bring the following documents for their spouse: Original Marriage Certificate with Seals, Original Birth Certificate with seals, Social Security card & a photo ID.
For first time DOD 100% DAV ID issuance the following documents are required: DD214, VA Award Letter/Award Packet, State or Federal Government Issued Photo ID and Social Security Card.
Customers will need to know their confirmation number to cancel or reschedule an appointment. Please keep a copy of your appointment confirmation in the event you need to cancel or reschedule your appointment.
If the information above does not pertain to your situation, please call the ID Card Office (904-542-1837) BEFORE arriving to ensure the DOD requirements will be met for your requested DEERS transaction to take place.

Utterly disgusting to take 3 1/2 hours to obtain an ID Card at NAS JAX.Would not accept a UNITED STATES GOVERNMENT MEDICARE ID as valid ID even though it is HARDER to get on of these than a Social Security Card.Front GATE, confiscated my expired ID over my protests . With it and my FL Driver License, I would have had no trouble.A terrible experience that took ALL DAY.Bad experience, bad memory and does not reflect well upon the United States Navy.

Calling on the phone is obviously a mistake, you can never actually speak with a PERSON....computer doesn't answer every concern or question. Interaction is required. Have a friend in NAS Hospital and son needs to come onboard to visit and or pick him up....sorry Charley...if not a dependent or have a sponsor with you to go through gate, out of luck. FIX IT.
